Our Rooted Foundation package is a great introduction to functional medicine!
This introductory package is designed to answer some of your questions about why you are feeling the way you do. It also gives us a great starting point to determine which direction to go if you decide to continue with our program.
Whats Included?
Initial Intake
You will complete an initial intake questionnaire, and spend 30 minutes with Dr. Pope to gather information regarding your health goals and history
Initial Female panel lab included (Access med lab)
Lab order will be sent in, You can make an appointment at the lab at your convenience! (Phlebotomy charges not included)
Report of findings
One hour report of findings call. This will be a great way to get a glimpse into potential root causes, and determine which advanced labs would best benefit you!
Comprehensive Bloom-well Blueprint program
The most comprehensive program we have ever offered. We will use advanced functional labs to explore the root of your hormonal issues, tracing your hormones and their metabolites upstream to figure out exactly where things get funky. This program is for those who are truly committed to taking back control of their life, and willing to make the necessary changes in their life to reach this goal.
With this package, you’re getting a doctorally-prepared health researcher personally working your case, from beginning to end, to learn more about whats going on in your body than you ever though possible. Then, we change hats and work with you to build a plan to put everything we learned into action, and coach you every step of the way!

10 total appointments with Dr. Pope
Comprehensive intake appointment
We will take a deep dive into your health history to explore and identify possible root causes of your problem
Dutch Plus, Thyroid panel and HBA1C included
You will complete the Dutch Plus advanced functional lab to dive deep into whats happening behind the scenes. This lab will provide detailed information about sex and adrenal hormones. We will also do a deep dive into your thyroid and blood sugar health, as these are foundational to full body hormonal health!
Fitbit and resistance band kit (or $120 lab credit!)
Report of findings visit with personalized Bloom-well Blueprint
Blueprint will include all patient lab information, an easy to read explanation of the significant markers, guides, with references, for managing these markers, and explanations for how they impact hormonal health. Blueprint also will include space for patient food journaling, supplement planning, and exercise log.
eight 30-minute follow-up visits
Scheduled twice per month, these give us the opportunity to continuously improve the clients plan, adjust when necessary and provide the accountability needed to stick with the plan.
Unlimited messaging through patient portal
Patient will have access to unlimited messages through the portal with a maximum 2 business day reply
